Bio

Dr. Cantwell is board-certified in Pediatrics, and sees both adults and children in his Complementary/Alternative Medicine practice.

He is a Quaker and seeks to integrate the physical, mental/emotional, and spiritual aspects of health. His book, Map of the Spirit: Diagnosis and Treatment of the Spirit, presents a universal framework of human spiritual development and seeks to explain the connection between spirituality and disease.  The book is available in paperback and ebook formats through www.mapofthespirit.com and Amazon.com.

Before starting Rising Phoenix Integrative Medicine Center, Dr. Cantwell was Lead Physician at the Institute for Health and Healing Clinic in San Francisco (1998-2012), one of the nation’s largest integrative medicine centers. His work in the CAM field has been recognized at the national level through multiple publications, lectures, and service on the first National Advisory Council for the National Center for Complementary and Alternative Medicine (NCCAM) of the National Institutes of Health (NIH) in Bethesda, MD.

Conventional Medicine Training

Dr. Cantwell graduated summa cum laude from State University of New York (SUNY)-Buffalo, NY with BA’s in Biology and Philosophy. He attended medical school at SUNY-Upstate Medical Center in Syracuse, NY and received his post-graduate pediatric training at the University of Minnesota Hospitals in Minneapolis, MN and the University of California-San Francisco (UCSF),CA. He is currently Board-Certified in Pediatrics.

Dr. Cantwell went on to earn his Masters Degree in Public Health (MPH) in Epidemiology at the University of California-Berkeley and was a Geographic Medicine Fellow at Stanford University.  Subsequently, he served in the Tuberculosis Control Branch of the Centers for Disease Control in Atlanta, GA and completed a post-graduate Fellowship in Pediatric Infectious Diseases at Stanford University and UCSF

Making Appointments

Current Patients:  We are currently changing our online scheduling system.  Please call our clinic at 415.831.4412 and speak with Dr. Cantwell’s Administrator.  Thank you for your patience during this transition.

Established Patients: (Those seen by Dr. Cantwell within the last 3 years) should schedule a “60 Minute Established” Appointment

New Patients: Can schedule a 90 minutes “Initial Visit” appointment by calling the office and speaking with Dr. Cantwell’s Administrator.  Initial visits cannot be booked with the online system.

Dr. Cantwell’s Administrator is onsite to respond to Dr. Cantwell’s office calls and emails on Mondays, Tuesdays, and Thursdays from 9:30 am – 6:00 pm.

Fees

Dr. Cantwell’s current hourly rate is $365/hour.

(Same rate for in-person and telephone appointments)

90 minute appointment: $550  (Initial visit & other in-depth visits)

60 minute appointment: $365  (Standard follow-up visit)

45 minute appointment: $275

30 minute appointment: $185

Payment Policy

Payment is expected at the end of the appointment. We accept cash, check, VISA, or MasterCard. There is a $25 fee for each returned check.

Insurance Policy

Dr. Cantwell does not accept any health insurance plans at Rising Phoenix Integrative Medicine Center. After their visit, (non-Medicare) Patients with PPO insurance will be given a superbill with appropriate CPT/ICD10 diagnostic codes for them to submit to their insurance plan to seek out-of-plan reimbursement (directly to the patients). Patients covered by Medicare or other Medicare supplemental or Medi-Gap insurance policies will be given only a simple receipt (without CPT/ICD10 codes) and will not be able to submit any claims to their insurance policies for Dr. Cantwell’s services at Rising Phoenix Integrative Medicine Center.

Cancellation Policy

Clients must provide four calendar days notice to either cancel or reschedule an appointment.  If less than four days notice is provided, clients will be charged 50% of the full fee for the appointment.

We require that all patients have a current credit card on file to use for billing “late cancellation” fees.
